Life Enrichment (Activities) Coordinator

This position may be full or part time depending on the right candidate's availability. Some weekends and rotating holidays will be required for this position.

Fallbrook Woods is seeking an energetic, compassionate and creative individual for the position of Life Enrichment Coordinator. The Life Enrichment Coordinator assists the Life Enrichment Director in implementing a creative, meaningful Life Enrichment program to meet the individual needs and interests of residents in the moderate to severe stages of Alzheimer’s/dementia. This is a “hands-on” position, in which the majority of time will be spent directly with residents, engaging them in a daily schedule of group activities and Life Enrichment programs. Musical ability is a plus in this position.

The following qualifications are preferred, but we will provide training for the right individual:

*Experience working in a healthcare setting

*Experience working with individuals in the mid to late stages of Alzheimer’s/dementia

*Experience working in the Activities/Therapeutic Recreation field

*Experience working as a professional caregiver (P.S.S. or C.N.A. preferred)

Recreation Coordinator - 32 hrs. per week (Benefit Eligible)

Francis Parker Memorial Home, offers adults a continuum of exceptional long-term care services, including residential nursing care and specialized memory care, assisted living and adult day health. We are a progressive, private, non-profit, Nursing Home & Assisted Living seeking a Recreation Coordinator  to join our team.

To provide activities of a physical, social, mental, and spiritual, nature tailored to the individual abilities, needs and desires of the population in order to enable the Residents to enjoy the highest quality of life.

General responsibilities include:

Set-up, implement and conduct large, small group and 1:1 recreation programs according to a monthly schedule (this includes out-of-home trips);

Motivate and encourage the individual’s participation in programs and activities located throughout the community

Encourage and provide assistance to maintain individualized leisure interest/needs and provide supplies as needed ;

Complete initial assessments within 14 days of Resident's arrival and follow up documentation and as indicated

Provide reports regarding each residents’ participation in the program

Minimum Requirements

B. A. or B. S. in Therapeutic Recreation, Creative Arts Therapy or in related field required or ADC;

Minimum 1 year experience in Long Term Care & Dementia Care

Working knowledge in Excel, Word, Power Point and related programs;

Drivers License in good standing; must be able to obtain CDL within 6 months of hire

Ability to move/lift/carry objects, approximately 25 to 75 and to occasionally assist in lifting or transferring residents weighing average 150 pounds;

Certification as an activity professional preferred
