For PUBLIC webinars, be sure to check the current webinar schedule before registering, to see what is being offered.

Also, it is VERY IMPORTANT that you provide your phone number in the space provided upon registering. Sometimes emails are blocked and I need another way of getting in touch with you to ensure you receive the log-in information for the webinar.

This is form is for individuals who wish to pay by check or money order and are interested in attending sessions per the Current Schedule.

PUBLIC Group Webinar Mail-In Registration Form This form is for groups who wish to pay by check or money order and are interested in attending sessions per the Current Schedule-along with individuals outside the group.

PRIVATE Group Webinar Mail-In Registration Form This form is for groups who wish to pay by check or money order and are interested in scheduling a private session for their group or facility. Only individuals from the group/facility may attend.
